Please bring the following items to your child's Kindergarten Signing appointment:

• Child's birth certificate

• Child's immunization record

• Parent/guardian's proof of residency (This can be a form of identification or a piece of mail verifying parent/guardian name and address.)

The Michigan Public Health code requires that newly enrolled Kindergarteners must show proof of full immunizations or a waiver from the Health Department by the first day of school.
